Child Medical Declaration

We will assess your child's medical needs and accommodate any changes we can make to make the activity as fun and engaging as possible. Please be aware that in some cases, it may not be possible for you (or a delegated adult) to leave the site during your child's participation in the activities being hosted by Goal Stars. Someone from our team will be in contact with you shortly to discuss your submission with you.
